  2. Perform test for both Intranet and Internet.
  3. Capture the screenshots of your test result.
  4. Fill Report Form.Provide the following information for further investigation.
    (1) Building / Room No.Provide the accurate location of your network issue
    (2) Contact NumberProvide contact number for follow up
    (3) Network MethodsSelect the network methods that you used when issue happened
    (4) Date & Time of Issues OccurredProvide both Date and time of issues Occurred
    (5) PlatformSelect the device(s) you used for the network connection
    (6) System OSSelect the operation system(s) of the device(s)
    (7) What issues happen when you have a network outage occurs?Select the type(s) of issue(s) of your network performance issue
    (8) Problem DescriptionDescribe the issue and provide as much details as possible such as affected website URL, APP name, etc.
    (9) Attach FilesAttach the network test result screenshots that are captured from STEP 3
