  1. Login to Facility Access Card Application
  2. Select “My Card(s)” on bottom navigation bar
  3. Locate the card by keyword or scroll to find in view
  4. Click “Report Lost” on the card
  5. Submit the form for report lost.
    1. Choose the “Request a replace card” if necessary
    2. Input the “Remarks”
    3. Submit the form by clicking “Submit”
